Which wiki for me?

What wiki software do you suggest given the following factors? (Sorry to ask the most popular question here, but I don't see an example where someone had the same needs/wants as me.)

- Open source
- Primary content stored in database
- Revision history on all content with diff capability
- WikiPedia-like discussion pages paired with content pages
- Hosted by us on shared server
- Per-page attachments
- Flexible access levels
- Embedded page objects:
    - maps from Google and/or other mapping source
    - content derived from external database
    - maybe others; a flexible, plug-in based facility
      would be a plus
- Supports form-driven input for cases where we will
  have multiple instances of a "page type". So, for example,
  if we have 500 appliances and we want to enter a page for
  each, I'd like to display a form that has input fields for
  the 10 or 12 characteristics of each appliance. The editor
  would complete the form and that would create the page.
- I'd prefer systems implemented in MySQL/PHP, but that's not
  a hard and fast requirement

Those are the major features I'm looking for. If some of them are available via plug-ins or extensions of some kind, that's fine.

  The wiki you'd want is MindTouch Deki Wiki.  It is Open Source, has all of the functionality you are looking for and then some.  It is available for download here: http://wiki.mindtouch.com/Deki_Wiki

Corey,

I looked at Deki Wiki yesterday, and I liked what I saw, but I saw posts here on WikiMatrix that said Deki Wiki can not be used on a shared server... Has that restriction been removed?

The wiki you'd want is MindTouch Deki Wiki.

Close, but I think Transom is looking for separate discussion pages and the ability to host it himself on a shared server:

- WikiPedia-like discussion pages paired with content pages
- Hosted by us on shared server

DokuWiki with the monobook theme would come close:

- Open source
DokuWiki is Open Source

- Primary content stored in database
DokuWiki uses a flat file database

- Revision history on all content with diff capability

DokuWiki is a small script that works well on a shared server

- Per-page attachments
A plugin that might take care of that: http://wiki.splitbrain.org/plugin:medialist but no near as nice as what DekiWiki offers on per page attachments.

Data plugin might come close but it is not going to create pages from the data like I've seen done with MediaWiki: http://wiki.splitbrain.org/plugin:database

DokuWiki is written in PHP. It does not use a MySQL database.

However, I do suggest trying out DekiWiki's hosted solution first: http://wik.is/ You can set up a free account and take it for a spin. The pro version is $60 a year until the end of the year and $120 a year after that. Their template system might be all you need to create the "form-driven input" you want for creating new pages (users would choose a template you created when building a new page). The menu system is beautiful, no need to create one as you go along - it automatically adds pages to the menu on the left and the attachments are per page. Another big plus is the easy to use WYSIWYG editor. The big drawback is the discussion pages (there are none), it comes with the ability to leave comments at the bottom of each page (which more less makes it look like a blog in my opinion).

I'm convinced I could sign companies up on wik.is all day long if there was a separate discussion and attachments page for each article that could be hidden from the public but that's not the case.

Thanks for your detailed reply. Very helpful!

I'll take another look at DokuWiki. Deki Wiki does look great, but we need the content on our shared host, and I can't change that.
