  I'd recommend you take a look at Deki Wiki.  Below I've addressed all of your requirements and how Deki Wiki fulfills them.

1. Free download and can be installd on a separate server.

Deki Wiki is Open Source and can be installed on a separate server.

2. Datastorage in a database, preferably MySQL

Deki Wiki user MySQL for storage as a default.

3. Support WYSIWYG either default or by plugin

WYSIWYG editor is setup by default

4. Good ability to change the look either by templates or some other way. Support treeview hierarchy.

We have a couple of templates with many variations of color that allow you to customize via CSS overrides, which are managed in the control panel. 

5. When you create a new page - I want it to be mandatory to select a category for the page and that the choices are pre-defined and expressed with radiobuttons or some other way.

In regards to categories, we have tags and we have hierarchies that you can sort your content.

6. I want to be able to block a page from editing.

Deki Wiki has granular permissions which allow you to restrict editing and viewing of pages

7. Older versions of a page must be available so no data are lost.

We version all of the pages and files and keep full forms of the page and file

8. Good full text search

Deki Wiki uses Lucene, an enterprise search engine to search the pages and the contents of files

9. Easy to insert and edit images.

Deki Wiki has intuitive dialog for inserting images, links, and extensions

10. If it has a map-function that allows users to set a point on the map with a link to a wiki-page it is a bonus.

Deki Wiki has integration with Google Maps which allow you to embed the map with an address and it auto points to the address.
