Wiki with form entry (prepared form[s] by site owner)?

Hello.

I am looking for a wiki, with form input.
The operator specifies what information must be entered where.
Form elements such as text boxes, text fields, check boxes, radio buttons, selection lists and file fields (upload)...

Useful would be a good search function.
Refine results with the search by one or more search fields.
Search Fields = Form Elements

Example:
Search in "Resolution" = many results.
Search in "Resolution" and "Manufacturer" = fewer results.
Search in "Resolution", "Manufacturer" and "Zoom" = even less results.

Good would be a rating system in several categories

In addition, a forum thread for each wiki page.

Is that possible?

afaik nothing that fits your exact description exists yet.  But your requirements as listed are  what FormLis (http://www.formlis.com/) aims to be.

Here is how it currently (Dec 30, 2010) compares to your requirements:

* Form Elements -- Yes.  Text boxes, checkboxes, radio, selection.
* File Upload fields --- Not yet.  Its in the plan.
* Search Function --- not yet. Its in the plan, but see the reporting capability (next point).
* Reports -- FormLis has a really powerful reporting (see the demos on the main site).  Collected form data is used to make tables and can have grouping, sorting, filtering, coloring, calculations, totals &c.
* Security --- not yet, its what I'm working on right now.
* Discussion Thread per page --- not yet, but its in the plan.

So I may not have all the features you need, but check back in a few months. If you want to get in touch (I'm at warrenwilkinson@gmail.com), I can inform you when features are added.

Here are my ideas. Optional all. The admin decides what he enabled or disabled.

Each visitor (registered or unregistered) may open up a new entry about a "thing", or uses an already created entry. Visitors should first look to whether an entry is already present.
This can be a restaurant, doctor, mechanic, automobile, celebrity, cereals, politicians, chewing gum and so on.

For different kinds of purposes.
* A review site for restaurants, doctors, hardware, cars etc..
* Search for the identity of a violent/inappropriate (criminal) Public servant (police officer etc.)
Attention, off topic Background:

In Germany, police are rarely convicted (even if there are evidence like photos or videos).

The unions are resisting identifiability. The judiciary does not attempt to identify them.
They bludgeon anonymous, protesters at demonstrations.
Demonstrators is it forbidden (under court penalty) to wear a mask.
It will be legally prosecuted as a "weapon", if someone is wearing a protector or similar (normal) protective clothing.
Police violence is legal, to protect themselves illegal. Some police officers wear quartz gloves (maybe "unofficially").

E.g. Camera/audiorecorder:

* Manufacturer
* Type Name
* megapixel resolution
* Optical Zoom
* product foto(s) [upload field for pictures]
* photo samples [upload field for pictures]

* Manual [upload field for pdf and other documents]

Doctors (or generally person):
* Gender
* Last name
* first name
* middle name
* City
* Zip Code

* No.

* Specialization (general practitioner, gynecologist, ear/nose/throat doctor...)
(* Height)
(* Weight)
(* Hair color)
(* Eye color)


Individual media, (in view mode) are shown in a window.
E.g. the product photo. Or a video in a player window.
The admin placed these windows, in the form.

In the normal mode (view mode) the user/visitor can download the media files.

OK, body data are irrelevant to a doctor.
This is just one example. For a partnersearch site are more likely.

Rating options:
* 5-star

* 10-star

* Grades.
Remember that these are different International.

It is easier, even if the admin can set note ranges.
In letters (USA) or numbers (Germany 1 to 6, Switzerland 6 to 1...). With additional text.
E.g. Germany:
Number: "1" / Additional text: "very good".
Number 6 / Additional text: "unsatisfactory".

Grades in different "school" subjects.

E.g. Restaurant:
- Cleanliness
- Ambiente
- Taste
- Friendliness
- Price
- Parking
And so on.

This arbitrary single notes, and a calculated overall score.


* Two point "Good/bad": Red/Green or laughing smiley/sad smiley or... (arbitrary graphics)
* three point like ebay (good, neutral, bad or three smileys or...).

Not just a single method. Let the admins decide.
If that is all too much work to integrate, then at least (or "for now") two or three point or point AND school grades, or two/three point as the minimum.

Gimmick:
Location information in a map (Google Maps API). Place a marker, or automatically from address (address fields).
The rating can be displayed in the map as markers.

Automatic Live Search, which makes suggestions for existing entries (reduce duplicate entries).
There are forums that suggest while entering the title existing threads.


* For each entry a separate forum.
The users can discuss together the "thing " (object or person or country, or ...).


* For each user a comment.
See in Google Maps. There you can rate and leave a comment.
As the admin wants:
[] A user may edit his comment.
[] A user may not edit his comments.

If he can not edit, he can make a supplement.
Time and date are automatically inserted.

Search function:

Full search over all data (boolean or "intelligent"). Or data fields are combined to narrow down results.

E.g. Restaurants:

"50" or "5" instead of 50374.

City: Cologne

Useful: Radius search.
5Km, 10Km, 15Km, 20Km.....

Given your description I'm not sure my product would be a good fit.  But something you could try out is Twiki with Twiki Forms.

It basically lets you 'stamp' a page with a form.  I used it at a company to stamp some IT pages about office servers with a form to specify OS, software it was hosting, etc.  It's also free, so it costs nothing to try. And you host it yourself so you have full access to the data.

These were many examples. What I want, I need to think about it.
Is there an aversion to the use of Formlis for such purposes?

FomLis is so far the most promising product.

Self hosting? On a personal computer? I do not have a server with a dedicated line.


EDIT:
Is FormLis a Script like phpBB ("install" on a webspace)?
I can't find a download.
